Nipah virus, a member of the paramyxovirus family, was first isolated and identified in 1999 when the virus crossed the species barrier from fruit bats to pigs and then infected humans, inducing an encephalitis with more than 42 40% mortality. At present there is no prophylaxis for Nipah virus.
In recent years, several paramyxoviruses have emerged in this manner. Rubulaviruses, which have been associated with abortions in pigs , have been isolated from these fruit bats, both in Australia and in Malaysia..
The virus family Paramyxoviridae, consisting of viruses possessing non-segmented, single stranded negative sense RNA genomes, is also the source of several highly contagious pathogens such as measles virus and mumps virus in humans and canine distemper virus in dogs
Paramyxoviridae is a family of viruses in the order Mononegavirales. Humans, vertebrates, and birds serve as natural hosts. There are currently 49 species in this family, divided among 7 genera. Diseases associated with this negative-sense single-stranded RNA virus family include measles, mumps, and respiratory tract infection
Nipah virus was identified in April 1999, when it caused an outbreak of neurological and respiratory disease on pig farms in peninsular Malaysia, resulting in 257 human cases, including 105 human deaths and the culling of one million pigs.
In Singapore, 11 cases, including one death, occurred in abattoir workers exposed to pigs imported from the affected Malaysian farms. The Nipah virus has been classified by the Centers for Disease Control and Prevention as a Category C agent
Some hundreds of years after measles virus crossed into man, the paramyxoviruses as a group have continued to be a source of emerging zoonotic infections: two new additions to the paramyxovirus family, Nipah virus (NiV) and Hendra virus (HeV) emerged to cause infections among humans at the end of the last century.
HeV and NiV were assigned to a new genus, the henipaviruses, based in part on both their broad host range and ability to cause mortalities in both humans and animals and their unique and distinctly large genomes size - 18,234 nucleotides for HeV , and 18,246 or 18,252 nucleotides for NiV Malaysia and NiV Bangladesh respectively - which are approximately 15% larger than other paramyxovirus genomes.
For HeV, the disease manifests in 4-20 days, whereas for NiV, the illness presents with 3-14 days. It is characterised by fever with acute encephalitis, or as an acute influenza-like illness leading to severe respiratory illness, or neurological manifestations.
Henipavirus is a genus of RNA viruses in the family Paramyxoviridae, order Mononegavirales containing five established species. Henipaviruses are naturally harboured by pteropid fruit bats (flying foxes) and microbats of several species.
Henipaviruses are characterised by long genomes and a wide host range. Their recent emergence as zoonotic pathogens capable of causing illness and death in domestic animals and humans is a cause of concern
In the first outbreak in 1994, HeV infected and caused mortalities in horses and humans In an effort to determine the reservoir species for HeV, a serological survey was carried out in eastern Queensland with sera collected from 46 species including 34 species of wildlife.
No antibody was detected in this initial survey, but in a second survey targeting flying foxes and birds, antibodies capable of neutralizing HeV were detected in the 4 mainland species of pteropid bats found in eastern Australia and virus was subsequently isolated from the reproductive tract and urine of wild-caught bats.
NiV appeared some 4 years later in an outbreak that primarily affected pigs and humans in peninsular Malaysia and Singapore , and was later shown to be closely related to HeV by immunological and molecular analyses .
Pteropid bats were the suspected reservoir host based on the similarities between HeV and NiV. Again surveillance of animal species identified neutralizing antibody to NiV mainly in flying foxes (Pteropus sp). Virus was isolated from urine of these animals and from partially eaten fruit under trees in which bats foraged.
In later episodes of human NiV infection in Bangladesh and India were characterized by a higher mortality rate and clear evidence of human-to-human spread . Respiratory symptoms were more severe and the fatality rate was more tha 71% .
In the Faridpur outbreak in Bangladesh in 2004, 77% of patients developed respiratory difficulty and the associated fatality rate was more than 74%. In addition, patients with respiratory symptoms were more likely to transmit the virus , and the case for the role of respiratory secretions in the human-to-human spread was further strengthened by the identification of NiV RNA in the respiratory secretions of infected patients.
In the more recent emergence of NiV in Bangladesh in early 2011, the mortality rate has exceeded 77%. NiV infects humans, bats, pigs and dogs in nature, and like HeV, other mammals replicate virus following exposure under laboratory conditions.
Henipavirions are pleomorphic (variably shaped), ranging in size from 40 to 600 nm in diameter.They possess a lipid membrane overlying a shell of viral matrix protein. At the core is a single helical strand of genomic RNA tightly bound to N (nucleocapsid) protein and associated with the L (large) and P (phosphoprotein) proteins, which provide RNA polymerase activity during replication.
Since 1994 the henipaviruses have periodically emerged from their animal reservoir in flying foxes to cause disease in human and animal populations. The recent emergence of these viruses coupled with the high mortality rate associated with henipavirus infections and the lack of any licensed prophylactic or therapeutic treatments, makes them agents of particular concern in the area of both human and agricultural biodefense
The bats themselves do not show any clinical signs of illness. No bats-to-human or human-to-human transmission has been documented so far. For Nipah virus, the domestic pig acted as an amplifying host. Transmission of Nipah virus occurs after direct contact with infected bats, infected pigs, and NiV infected patients. NiV transmission in humans also takes place through exposure to body fluids - secretions, excretions, etc
Molecular biology studies have shown that this new virus have a similar genomic structure, but as their genomes contain some 2,000 nucleotides more than previously studied paramyxoviruses, the Nipah virus have now been classified into a new genus, Henipaviruses, within the family Paramyxovirinae.
Nipah virus is one of several viruses identified by WHO as a likely cause of a future epidemic in a new plan developed after the Ebola epidemic for urgent research and development before and during an epidemic toward new diagnostic tests, vaccines and medicines

Neurological signs include reduced levels of consciousness, segmental myoclonus, areflexia, hypotonia, and abnormal doll's eye-reflex which develop in these individuals within a week of fever onset.

The outbreaks in Malaysia, Bangladesh, India, and the Philippines involved the loss of life and diminished the earning potential of survivors due to long-term disability demonstrating a clear economic impact of the NiV outbreaks. Because NiV is highly infectious to humans by aerosols, and is life-threatening to humans and agricultural animals with a high CFR, NiV is classified internationally as a biosafety level 4 (BSL4 or P4) agent..
Over half of cases in Bangladesh are due to human-to-human transmission.
